Connie has been a part of the Glengariff team for over ten years, beginning as the assistant director of admissions, to director, and now clinical liaison. As part of her many responsibilities, Connie builds relationships with hospitalists and practitioners in the community, communicating with admitting patients and their families, and ensuring what the team refers to as a “warm handoff” in terms of provided care. Connie is often the first professional member of the Glengariff team who patients will meet when beginning their transition from acute to subacute care. She provides patients and their families, with education, interviews, tours, and most of all, support to assist them through the often trying time of transitions between levels of care. Prior to beginning with the Glengariff team, Connie served as an operating room director for a metropolitan hospital and manager for a private surgical practice.
