When Nancy D. was admitted to Glengariff Rehabilitation and Healthcare Center from North Shore University Hospital on July 3, 2025, she arrived in a significantly debilitated state. Due to a combination of generalized muscle weakness, a pressure ulcer, and severe unsteadiness on her feet, Nancy was fully dependent for all care. She required the use of a Hoyer/mechanical lift for all transfers, as she was unable to move independently. At the time, she had been living alone at home but was completely reliant on others for all Activities of Daily Living (ADLs), including mobility, grooming, dressing, and toileting.

Upon admission, the interdisciplinary team at Glengariff—including physical therapy (PT), occupational therapy (OT), nursing, and wound care specialists—immediately initiated a comprehensive and individualized rehabilitation program. Their goal was not only to improve Nancy’s mobility and strength but also to support wound healing and help her regain as much independence as possible.

Through consistent, progressive, and targeted therapy sessions, Nancy began to show steady and encouraging progress. The therapy team, including Marc from physical therapy and her dedicated occupational therapists Stephanie and Khaleda, worked closely with her to rebuild her strength, endurance, and functional independence. Their support, combined with the clinical attentiveness of Nina, LPN, and CNAs Norma and Claudette, played a critical role in Nancy’s transformation.

By the time of her discharge on September 24, 2025, Nancy had achieved remarkable rehabilitation milestones. She was fully independent in bed mobility and transfers and could ambulate up to 150 feet with supervision—an extraordinary achievement considering her initial reliance on a mechanical lift.
